Our Vision
Axis Community Health is a health and wellness center offering medical, mental health, addiction counseling and health education services for everyone in the community.
Our Mission
The mission of Axis Community Health is to enhance the physical, emotional, and
social wellbeing of individuals, families, and the Tri-Valley community as a
whole through the provision of services that promote wellness and positive
quality of life.
Our History
Axis Community Health began in 1972 as a health center for low income children in Eastern Alameda County who had no other access to health care. Over the past thirty years, Axis has grown to include medical care for all ages. We have also added mental health counseling services, addiction recovery programs, court-ordered courses, a WIC nutrition program, and school and community-based health education services.
Until February 2004, we were known as Valley Community Health Center. We changed our name to Axis Community Health in an effort to increase our community visibility and eliminate the confusion that existed in the community between our organization and ValleyCare Medical Center, the local hospital. Although our names were similar, Axis has never been affiliated with ValleyCare.
Organizational Description
Axis Community Health is a nonprofit community organization that provides a wide range of medical, psychological and social support services for residents of the Tri-Valley area.
Axis is the sole provider of medical care for the indigent and uninsured residents of eastern Alameda County.
Our services include:
Adult, pediatric and geriatric medical care
Obstetrics, gynecology and family planning
Mental health counseling
Drug and alcohol recovery services for teens and adults
Court-ordered DUI classes
Domestic violence/anger management programs for teens and adults
WIC nutrition program
Community and school-based health education services

Foundation
Axis Community Health depends on the generosity of the community to provide our
many services. The Foundation for Axis Community Health was created to enhance
community support for our services, carrying out a variety of activities that
generate support for our organization.

Our Staff
Axis employs a staff of 90 that includes physicians, nurses, mental health professionals, drug and alcohol counselors, registered dieticians, health educators and a variety of support personnel. Axis also has an intern program for post-master's level mental health professionals.
Our staff is multi-lingual and provides services in a variety of languages. In order to ensure that services are provided in a culturally-sensitive manner, our staff is representative of a variety of cultures and backgrounds.

Community Need
Axis is the sole provider of medical care for the indigent and uninsured residents of Eastern Alameda County. Axis is also the only provider of affordable outpatient drug and alcohol recovery services in the Tri-Valley. If our services were not available, Tri-Valley residents in need of affordable medical care would have to travel thirty miles to Highland Hospital in Oakland. Those in need of affordable drug and alcohol services would have to travel to Hayward or Oakland for comparable care.

Who We Serve
Axis provides medical care for more than 10,500
Tri-Valley residents. An additional 2,000 local residents are
served through our Behavioral Health Program.
The vast majority of Axis's patients and clients are Tri-Valley working families who do not have health insurance plans. More than 50% of our medical patients are non-English-speaking, with Spanish being the predominant language. 62% of the patients live in Livermore, 23% live in Pleasanton and 9% live in Dublin. The remaining 6% reside in surrounding communities.

Why Axis Needs Community Support
Our annual budget is $6,000,000. This support comes from a variety of sources, including local, county and state support, foundation grants, community donations, and patient and client revenues, including Medi-Cal and Medi-Care. Public support does not cover the cost of providing care, so Axis relies on the generosity of the community to meet the need for services.
Community Partners
All human services in the Tri-Valley must deal with extremely limited resources in meeting the needs of a growing population. While this presents huge challenges, it also presents an opportunity for providers to work together so that resources can be used as efficiently as possible. As a result, organizations in the Tri-Valley collaborate on an ongoing basis to eliminate duplication of services and ensure that residents are referred to appropriate locations for assistance.
Axis is an active partner in this network. Because we are the sole provider of healthcare services for the Valley's uninsured population, we are in the unique position of serving as the focal point for many Valley residents. Through our comprehensive enrollment and eligibility process, we are able to quickly identify families who need the additional support of non-medical services such as housing, food and mental health services. We direct patients to the essential services they need, including our in-house mental health, behavioral health and WIC nutrition programs, as well as to external providers for services such as emergency shelter, employment training and childcare.
Axis also collaborates with local school districts to provide health education, drug and alcohol abuse prevention and mental health counseling services. In addition, we work closely with local courts and law enforcement agencies to provide court-ordered services in the areas of drug and alcohol recovery, anger management and domestic violence.
Although Axis has a well-established network among Tri-Valley human service providers, school districts, law enforcement agencies and local courts, it could not survive without the support of the community. As public funding for human services dwindles, community support becomes even more critical for Axis to continue its services. From the crumpled dollar bill left in the donation box by a grateful senior citizen, to the $25,000 grant we recently received from the SBC Corporation for the purchase of computer equipment, each donation is critical to our ability to continue meeting the needs of Tri-Valley residents.
