Enrollment Services
Our enrollment team is available via telephone, and in-person Monday – Friday to help patients enroll in Covered CA, Medi-Cal, CalFresh, Medicare, and HealthPAC. Axis also welcomes patients who are looking to pay for all their services. Please call 925-462-1755 to make an appointment.
In order to complete the enrollment process without contact, patients can transmit documents to Axis via a secure, encrypted link that Axis will provide you with. You will need to provide your last pay stub to verify year to date salary amount and if receiving unemployment, an award letter to enter the current income. You may also drop off any necessary documents in person as directed by our Enrollment Staff. After we receive your documents, a Certified Enrollment counselor will follow up with a phone call to verify your information.
